INITIAL ASSESSMENT
Engagement and assessment are the early stages of the generalist intervention model
and lay the foundation for the work to be done with a client. Consider an initial
assessment of a client that you would provide in a social work agency. You might
prepare a structured questionnaire or rating scale for the client to complete, and you
might conduct an extensive interview to learn about their history and the biological,
psychological, social, spiritual, and cultural factors influencing them. How might this
process look different in a healthcare setting? What challenges could there be to a
comprehensive assessment?

CARE PLANNING
Following assessment comes care planning, in which collaborative goals are developed
along with a method for achieving them. The goals are directly related to the key areas
of need found in the assessment.
If you are working with a client one on one in an agency, the care planning process is
likely an intimate one with the client, but in healthcare more people may be involved.
That is because the care plan not only includes the social work goals but also the goals of
other disciplines who may be collaborating with you to treat the patient, such as
physicians, occupational therapists, and nurses. It may also involve family and caregivers
who have particular insight into the patient.
In this Assignment, you engage with the topic of care planning in a particular healthcare
setting of interest.
To prepare:



Review the Learning Resources on care planning.
Select a healthcare setting of interest—either a new one or one you have
identified previously. With that setting in mind, consider the role of the social
worker in creating a care plan and the relevant factors and elements of a care
plan in that setting.
Reflect on the challenges a social worker might encounter working with other
professionals in the care planning process.

Submit a 2- to 3-page paper in which you:




Identify a healthcare setting / area of focus of medical social work that you
are interested in.
Describe the components of care planning within that setting.
Identify and explain the key factors that must be considered in the care
planning process. Include the role of assessment in developing the care plan.
Explain the roles of a medical social worker in creating a care plan. Be specific.

Finally, explain challenges a medical social worker might face working with
other professionals involved in care planning.
