Hello class, so this week my clinical experience was interesting. It was a little different than my previous clinical experiences because it involved seeing young adults as well as younger children. In the previous offices there has not been a lot of children that go for checkups. So, dealing with the parents of the patients was also a little different for me as well and adds a little more pressure because sometimes parents want answers right away and don’t like it if you aren’t sure about something I understand since I am a parent as well. Overall, though it was a good experience this week, because my preceptor took the reins most of the time as this was my first week in the office.
2.) Describe the assessment of a patient, detailing the signs and symptoms (S&S), assessment, plan of care, and at least 3 possible differential diagnosis with rationales.
S.M 15-year-old female patient came in with a slight fever of 101 fare height as well as headache, sore throat, and body aches. Upon assessment of the patient her forehead was warm to touch, her throat was red and swollen and when she swallowed, she had pain. Upon inspection of her nares both were not equally patent and there was congestion apparent. For the plan of care, the patient was told to take acetaminophen or ibuprofen to help with the patients fever as well as the body aches the patient was experiencing. If the Tylenol or the Advil do not help with the sore throat the patient was told to take Mucinex spray for pain. Patient was also prescribed Osetlamivir 2 times daily one in morning and in the evening for 5 days. Some of the differential diagnosis that are common are upper respiratory tract infections, some of the common symptoms with the FLU include fever as well as body aches. Another one is infectious mononucleosis; similar symptoms include fever sore throat and extreme fatigue. Covid-19 is also a common differential diagnosis as this has similar symptoms as fever, chills, and muscle body aches.
3.) Mention the health promotion intervention for this patient.
The health promotion for this patient will include, to make sure they get a lot of rest and sleep because this will help their body recuperate faster. Patient was also told to hydrate themselves with lots of water and Pedialyte if necessary, because the fever makes the children loose fluids more quickly. Patient was also recommended that if they feel the fever coming on to make sure to they put cold compresses on the inguinal areas, under armpits, thigh region, as well as the forehead. As well as to make sure to not wear excess clothing or too many blankets when she sleeps because it can raise the temperature.
4.) What did you learn from this week’s clinical experience that can be beneficial for you as an advanced practice nurse?
From this week’s clinical experience, I learned a new health care system I had not worked with before as well as working cohesively as a group with a new team and a new preceptor. I learned what was expected of me by the end of the semester and I am going to work to fully achieve all those expectations and achieve more as well.

Currently available FDA-approved antiviral medications include neuraminidase inhibitors (NAIs) for example oral oseltamivir, inhaled zanamivir, and intravenous peramivir (Uyeki TM, 2019). In this systematic review, we chose to include both antipyretic drugs and physical cooling when assessing the effects of fever therapy (Garg S, 2019).
