What two patient care delivery models could be considered for use within the clinical healthcare environment you identified? Two patient care delivery models I chose for a Postpartum unit were Team Nursing and Total Patient Care. Describe how each model would support safe, high-quality, person-centered care. With Team Nursing, there are other healthcare personnel who would be able to participate in the care of the patients. As the nurse leader, delegation of tasks is important to make sure the day runs smoothly, tasks get done, and patients are cared for. With Total Patient Care, the RN would have one patient that would be total patient care, and that RN would be responsible for that patient. This patient would be a new admit that way the RN knows the vitals, can establish rapport with the patient, knows what is going on with this patient, and provides education upon admission. Or it could be a high acuity patient that requires total patient care from the RN that is competent to care for the patient like a postpartum mag patient. I do feel like both patient care delivery models have their pros and cons. Team Nursing Care helps when there is a high volume of patients, and assigning tasks and having others help in the care of patients is beneficial.
What decision-making model would you apply to facilitate the selection of the model best suited for the environment, and what steps would you take to apply the model? Nurse leaders who have a Master of Science in Nursing (MSN) can use the synergy model to ensure that patient needs, and nurse competencies are aligned. This ensures that the most appropriate nurse with the necessary skills is assigned to care for patients who require those specific skills. By picking RNs with the necessary skills to care for certain acuity patients is something that a nurse leader should consider. This allows the patient to be properly cared for and this would fall under Total Patient Care as well, so that one RN could have this one patient to care for that they are familiar with. This model could also ensure that delegation is done properly and that the staff that are assigned to certain duties are competent to perform these tasks.
What leadership competencies will be needed to implement the new model? To implement the new patient care delivery model, the nurse leader would need to possess several leadership competencies. Communication and Relationship Building skills would be essential to effectively communicate with staff and patients, build relationships, and establish trust. Leadership skills would be necessary to make decisions, delegate tasks, and inspire and motivate staff to provide safe, high-quality care. Knowledge of the Health Care Environment and Clinical Principles would be crucial to understand the specific needs of patients and ensure that the care provided is evidence-based and up-to-date. Professionalism would be required to maintain ethical standards and act as a role model for staff. Finally, Business Skills and Principles would be necessary to manage resources, track outcomes, and ensure that the new model is sustainable and financially feasible.

The two patient care delivery models that I would consider for use would be the team nursing care model and primary nursing care model’s. Team nursing would support safe patient care by allowing the nurse leader to create safe patient assignments based on staff knowledge, expertise and experience. Creating assignments this way allows for safe patient care by having the more complex patients assigned to nurses who are better equipped to care for them, and gives newer nurses lower acuity patients to safely care for while building their skill set and knowledge. In this care model nurses typically care for a fewer number of patients promoting higher quality nursing care as well. They have more time to spend with fewer patients allowing for more nursing interventions to be implemented into care as well as EBP and QI strategies. Primary nursing care model also supports safe patient care by having a primary nurse assigned to the patient. The primary nurses spends quality time caring for this patient, and putting together a care plan for them too. This allows the alternative nurse to provide care according to the plan that was developed by the primary nurse. There is less guesswork in what the patient needs when there is a primary nurse, and this leads to high quality outcomes as well as allows the primary nurse to develop a relationship with patient.

I would utilize the synergy model to facilitate the selection of the model best suited for the environment. This model considers many different aspects of patient care allowing for recognition of various areas of needs on a unit. This model can be used to ensure that both nurses and patients are having their needs met on the unit. I feel using the synergistic model provides the most comprehensive assessment of both nurse, patient, and unit needs. Therefore this model would best aid in the selection of an appropriate patient care model for the unit. Steps that I would take to apply the model would include utilizing it for creating job objectives for staff members, performance criteria ect. Once I had all the necessary information needed I would be able to determine staffing ratios, staff productivity, and other elements that would give information helpful to the appropriate patient care delivery model to implement.

What leadership competencies will be needed to implement the new model?

Some of the leadership competencies that would be needed to implement the new model would include transformational leadership/ the ability to influence and empower other nursing staff to utilize the model. Clinical inquiry would be needed to continually assess where improvements in the care delivery model are needed. Other things such as collaboration, clinical judgement, and patient advocacy would be needed to maintain the success of the models implementation into the unit. These competencies should be displayed by the nurse leader for other nurses to view and hopefully desire to emulate going forward.

In a primary nursing care model, assigning a dedicated nurse to each patient is recommended to ensure high-quality care and continuity. This nurse will coordinate and oversee the patient’s care throughout their stay. They will work closely with the patient, their family, and other healthcare providers to ensure their needs and care are managed correctly. This model emphasizes the importance of building a solid nurse-patient relationship, which can lead to better outcomes and an overall positive patient experience. By having a primary nurse, patients can feel more confident and secure in their care, knowing they have a dedicated healthcare provider familiar with their unique needs and preferences.

In the total patient care model, one nurse is responsible for the patient’s complete care, which includes administering medication, monitoring vital signs, assisting with personal care, carrying out physician’s orders, and communicating with other healthcare professionals. This model enables the nurse to deeply understand the patient’s health condition and needs, leading to a more personalized care approach. It also allows the patient to build a rapport with their nurse, fostering a trusting and supportive relationship to enhance healing.

To ensure a well-informed and logical decision, the rational decision-making model will be utilized to assist in selecting the primary nursing care model. This model provides a systematic and structured approach to decision-making, which involves several steps. The first step is to identify the problem and establish the goals and objectives that need to be achieved. Next, collect all relevant information to evaluate each available option. Once the information is gathered, the advantages and disadvantages of each option will be carefully considered to determine which one aligns best with the goals and objectives established earlier. After the option is selected, an implementation plan will be developed, and the decision will be implemented. Finally, the decision will be evaluated to determine if it effectively achieved its goals and objectives.

What leadership competencies will be needed to implement the new model?

Advanced Practice Nurses should conduct a thorough self-assessment to implement the new leadership model. This involves evaluating their leadership skills, communication, critical thinking, problem-solving, decision-making, conflict resolution, team building, clinical knowledge, and resource management. By identifying their strengths and weaknesses, nurse leaders can improve their leadership skills, provide quality care, promote a positive work environment, and achieve successful outcomes.

Great insight in your post! I really appreciated how you described the leadership competencies needed to implement the new model. As MSN-prepared nurses, it is important that while we take that next step above our BSN-prepared selves, that we not forget simple nursing aspects such as collaboration among the team, the use of clinical judgement, and always remembering to advocate for what’s best for the patients. We will set examples for staff nurses and if we display these leadership skills, they will follow in our steps which will have a positive impact on patient care.
