Oh, thank goodness you’re here. That was a rough night. Dwayne is a 28-year-old male transferred to the E.D.
via EMS. He was found lying on the pavement in the parking lot of his friend’s house at 0615. Of course, it was
snowing, so he laid on wet pavement for who knows how long. He is confused, mumbling, and smells strongly
of alcohol. He has a 16 G IV to left antecubital started prehospital. We started IV infusion of thiamine, folate,
and a multivitamin in NS. We just protocoled some lab work, and the E.D. provider is seeing him now.

I just got off the phone with the communications center. EMS is out on a call for a school bus accident. It
sounds bad. They hit a patch of ice and slid out of control, running into a brick church. We need to get
these rooms emptied to prepare.
Your staff includes Zac and Danielle, experienced RNs with 9 years of critical care experience; Sarah, a
new nurse with 6 months of experience; and Robert, an experienced UAP.

Yesterday, Elijah woke up saying his ears hurt. We had taken him swimming over the weekend, and
he said he had water in them. He kept asking us to turn up the television, and he’s been sticking his
fingers in both ears, even though we tell him not to. Now he says the right ear hurts, and the left ear is
swollen and red on the outside. He had tubes put in his ears about eight months ago. He hasn’t been
eating much either, and he can’t afford to lose weight. I don’t think he has a fever. I think he’s also
due for some shots. Elijah seems so bright…can you tell us if he is up-to-date on his development?
