one of the activities I got to be a part of for the first week was entering errors into Q pulse which is a system that keeps
track of errors/ FDA reportable incidents. My preceptor had 2 incidents that she had to submit to the FDA which
consisted of entering the product number, the location the product was sent to and if it was transfused or not. The root
cause of this was because lab techs misinterpreting the paperwork.

After dealing with the errors me and my preceptor came up with a case study quiz for her department with similar
scenarios of the errors that we just had to enter into Q pulse. The reason we did this was to see how they would
respond to them the second time around and hopefully get them to see where they went wrong the first time. The 2nd
solution was coming up with a memo for her team to read and sign that they acknowledge the issue at hand and will
obtain better results moving forward.

Another activity I got to be a part of was a change plan and impact assessment due to the supervisor’s staff using
expired saline and not having the right ph documented in the SOP. we compared with other state superior’s
(Wisconsin, Michigan and Indiana) to identify the percentage of saline that would work for Illinois. With making this

we worked on a Nonconforming Review Form. Due to the temperature alarm going off in Isensix which is a monitoring
system to keep track of all the refrigerators and deep freezers that hold products for the Labatory. We had to give a
brief discerption of the issue and the rationale of the disposition with the inventory being rare, but the units maintained
their frozen state. we also had to do the risk assessment going over the SOPs to ensure the hemolysis of frozen
RBC’s was a main concern.

Me and my preceptor submitted a KPI (key performance Indictor) to her boss. KPIs tell you where exactly your team
stand with how good you are at attaining your department goals and making sure they align with the overall blood

Due to project fusion with the other states (Wisconsin, Michigan, Indiana) my preceptor ordered a new centrifuge for
the lab. We had to fill out a change control form on account of project fusion with the other states trying to operate with
the same documents and equipment as Wisconsin. The change control form would be analyzing the process and
making corrective and preventive changes to stop recurring issues between states. I also verified that the SOP
matched the instruction manual.

members to the workshop since she had an incident with her team misinterpreting the ABO compatibly for transfusion.
