Treatment Plan
Through radiographic and clinical examination, which lead to the staging of my patient as having stage III periodontal disease, I will recommend and implement non-surgical periodontal therapy, NSPT and provide specific homecare instructions that will support healing to the periodontium. This procedure is intended to remove subgingival calculus deposits and disrupt and eliminate bacterial colonies that have formed (Smiley et al., 2015). MGN will be informed to return in four to six weeks for re-evaluation post NSPT to see how periodontium responded to procedure (Periodontitis, n.d.). Three-month periodontal maintenance will proceed to assist in preserving MGN’s oral health (Journal of Periodontology, 2011).
The preventative service that I will also recommend is a 5% sodium fluoride varnish to be applied every six months to assist with caries control and prevention (Weyant et al., 2013). I will also recommend a prescription strength 0.05% fluoride toothpaste to use at home (Weyant et al., 2013). An antimicrobial rinse post periodontal therapy will also assist in the healing and reduction of biofilm, as well as a Biotin rinse to assist with stimulating salivary flow due to her xerostomia (mouthrinse, n.d.). An electric toothbrush, flossing and Waterpik are other recommendations that will effectively assist in the removal of biofilm and gingival inflammation reduction (Home oral care, n.d.).
MGN is scheduled to return and see the dentist to address her active caries. Adjunctive periodontal therapy recommendation would be a periodontal referral for consultation regarding gingival graft on teeth #20 and #29 (Periodontitis, n.d.).
The goals for my patient include arresting the acute periodontal disease. This intervention will assist in the retention of her natural teeth; it will also encourage behavioral changes that will progress her home care, promoting improvements to her overall oral health.
