The Uninsured in the US

The nation’s uninsured rate reached a record low in 2022 of 8%. This is primarily due to the passage of
the America Recovers Plan which enhanced Marketplace subsidies, along with the continuous
enrollment provision in Medicaid, and several recent state Medicaid expansions (National Health
Interview Survey, 2022). Most of the uninsured are the working poor, low-income families that have at
least one worker in the family, but earn too much money to qualify for Medicaid, but cannot afford
health insurance.
Characteristics of the Uninsured
Hispanics and African Americans are disproportionally uninsured. The chart below demonstrates that
Hispanics have the highest rates of being uninsured. Males more than females and young adults tend to
have higher uninsured rates. This could be because many young adults are willing to take the risk they
will not need health insurance, or they cannot afford it. The less educated and those who are foreign
born have lower rates of health insurance coverage. States located in the south have higher rates of
uninsured citizens, primarily because many southern states have not expanded Medicaid coverage.
Reasons for Being Uninsured
Insurance is unaffordable.
Insurance is unwanted.
A period of transition is happening.
Time is scarce.
Information about insurance is scarce.
Employer does not offer a health plan
Employer offers a health plan, but employee is not eligible as a part-timer
Employer offers a plan and employee is eligible, but employee does not purchase because too expensive
Cannot find a plan that meets their needs
ACA and the Effects on the Uninsured
The Affordable Care Act (ACA) expanded Medicaid coverage to nearly all adults with incomes up to
138% of the Federal Poverty Level ($17,774 for an individual in 2021) and provided states with an
enhanced federal matching rate (FMAP) for their expansion populations. This has greatly expanded the
number of people who now have health insurance via Medicaid or the ACA Market Exchange.
Thirty-nine states and DC have expanded Medicaid coverage (as of 11/22). Those who have not
expanded Medicaid are mostly states in the South and Great Plains. Medicaid expansion remains a
controversial, political topic over a decade since the passage of the ACA.
Consequences of Uninsured
Not having health insurance leaves people without protection against catastrophes, higher out-of-pocket
prices, poor health, fewer services when care is used and worse outcomes. Being uninsured can lead to
higher levels or stress, especially financial distress, and higher death rates.
Safety Net for the Uninsured
Since 1985, it has been illegal for a hospital emergency department to deny care to anyone requesting it.
This is because of the Emergency Medical Treatment and Labor Act (EMTALA.) This federal law requires
anyone coming to an emergency department to be stabilized and treated, regardless of their insurance
status or ability to pay. Hospitals must provide medically appropriate screening, and treat or stabilize,
before transferring patients without insurance to a facility that can treat the patient. Most of this care is
financed from municipal budgets, Medicaid subsidies or Medicare.
