Community and Target Aggregate
• Describe the population demographic being served by this organization.
The Nicklaus Children’s Hospital, previously called Miami’s Children Hospital, primarily
serves pediatric patients in the regions within Miami and its environs (Nicklaus Children’s
Hospital, 2023). These patients range from infants to adolescents in the region. The current
community teaching project targets parents, teachers, caregivers, and other key stakeholders
involved in promoting children’s well-being in the community, specifically in Miami Dade
County, where the prevalence of childhood obesity is significantly high.
Social Determinants of Health (SDOHs):

Identify actual and potential SDOHs that impact the demographic served by the
nonprofit organization.
• Use the following data sources or similar ones to find relevant data and identify
SDOHs: 2023 County Health Rankings National Findings Report, CDC Behavioral
Risk Factor Surveillance System, National Center for Chronic Disease Prevention and
Health Promotion, and United States Census Bureau QuickFacts.
Several social determinants of health impact the pediatric and children demographic
population served by the Nicklaus Children’s Hospital in Miami Dade County, particularly
regarding childhood obesity. An actual social determinant of health impacting the population
is socio-economic factors, with low-income families in the community being unable to provide
healthy nutrition to the children, thus resulting in high prevalence obesity rates. A potential

SDOH that arises from this is unequal access to nutrition and fitness programs (Teens who are
Overweight or Obese, 2023). The other actual SDOH that impacts the pediatric and children
demographic population with regards to obesity is health literacy, with a significant population
not accessing resources about the management and prevention of childhood obesity.

The proposed education topic for the community teaching project is “Promoting Childhood
Well-being: A comprehensive guide to prevention and management of childhood obesity in
Miami-Dade County.”
Rationale of Topic:
• Explain the reason for your choice of education topic based on your research.
Core rationales for choosing “Promoting Childhood Well-being: A Comprehensive Guide to
Prevention and Management of childhood obesity in Miami-Dade County” as the community
teaching project topic is owing to the high prevalence rates of the health issue in the targeted
community; thus, a need to use stakeholders’ education as an intervention to address the health
issue. The other rationale for the topic choice is its alignment with the hospital’s mission and
vision, which primarily focuses on promoting comprehensive and holistic healthcare for the
children of Miami State and its environs through appropriate health promotion and prevention
interventions (Nicklaus Children’s Hospital, 2023).

regarding this health issue is to apply a multi-component approach towards reducing childhood
obesity from 19.7% to 15.5%.
Relevant information from the HP2030 websites that will assist with my presentation includes
the nationwide childhood obesity data and statistics, guidelines, and measures to address
childhood obesity in the country. Other information that will guide the presentation includes
action tools to address childhood obesity and areas to be prioritized in the community teaching
project.
