develop a mentor experience evaluation tool for your mentee to complete. You can develop a survey, a questionnaire, or short-answer response document or electronic tool. At a minimum, your evaluation should include at least six prompting questions and feedback comment areas. Though not exhaustive, the following is a list of suggested prompts you may incorporate into your tool:
Mentor availability and frequency of meetings
Mentor communication (frequency and clarity)
Mentor demonstration of interest
Mentor knowledge, competency, and guidance toward goal achievement
Mentor attitude of advocacy and demonstration of integrity
Value, efficacy, and clarity of mentorship activities (e.g., goals, guidelines, timeframe, personal development, feedback)
Most/least beneficial activities
Completion of goals planned
Mentor guidance with networking
Most beneficial personal development or change and overall personal growth
Recommendations for mentoring partnership efficacy and improvement
Overall value of the mentoring program.
